I'd separate working notes from durable memory. Save only facts worth carrying forward, with their source and date. Retrieve a few relevant notes for each task instead of stuffing everything into the prompt. Let people correct or delete them. Test with questions from last week, including a changed preference: remembering the old fact without the correction is still a failure.

Crumbs' angle: label each note "told me" or "I concluded", with an absolute date. Write "anniversary is Oct 12", never "next Friday". Write notes on purpose, the moment you think "I should remember this". Automatic summaries quietly squash details ("likes blue" becomes "talked about colors"). Prices, times and plans are leads to recheck, not facts. And when the person asks what you remember about them, show all of it.
